How do I sign up?
1. Go to: https://hamiltoncounty-tn.finalforms.com/
2. Locate the parent icon and click NEW ACCOUNT below.
3. Type your NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.
NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration.
If you do not receive an email, then check your spam folder. If you still can not locate the FinalForms email,
then email support@finalforms.com informing our team of the issue.
4. Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman.
Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
5. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
6. Click REGISTER STUDENT for your first child.

Registering a student
What information will I need?
Basic medical history and health information. Insurance company and policy number. Doctor, dentist, and medical specialist contact information. Hospital preference and contact information.
How do I register my first student?
IMPORTANT: If you followed the steps on the previous page, you may Jump to Step number 3.
1. Go to: https://hamiltoncounty-tn.finalforms.com/
2. Click LOGIN under the Parent Icon.
3. Locate and click the ADD STUDENT button.
4. Type in the LEGAL NAME and other required information. Then, click CREATE STUDENT.
5. If your student plans to participate in a sport, activity, or club, then click the checkbox for each. Then, click UPDATE after making your selection. Selections may be changed until the registration deadline.
6. Complete each form and sign your full name (i.e. ‘Jonathan Smith’) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the next form.
7. When all forms are complete, you will see a ‘Forms Finished’ message.
IMPORTANT: If required by your district, an email will automatically be sent to the email address that you provided for your student that will prompt form your student to sign required forms.
How do I register additional students?
Click MY STUDENTS. Then, repeat steps number 3 through number 7 for each additional student.
How do I update information?
Login at any time and click UPDATE FORMS to update information for any student.

Middle School Cross Country is open to all students in grades 6-8. New runners are welcome; no experience is necessary!
Football
East Hamilton Football Information
Thank you for your interest in our football program and allowing your child the opportunity to come out for the team. Here is what the players need to know before coming out for football.
· The school will supply each player with a helmet, shoulder pads, knee pads, and practice jersey.
· Each player is responsible for a 5-piece girdle, practice pants, or practice pants that have pads included. Also, a mouth piece is needed if the player has braces and shoes.
· The cost for each player will be $150 but this will include a pair of shorts, shirt, hoodie, equipment bag, and athletic fee. If this is an issue, don’t hesitate to discuss this with me. I don’t want this to have any bearing on coming out for the team.
· All players must have a current physical (dated after April 15, 2025), concussion form, cardiac arrest form, and emergency form. All paperwork is available on TSSAA.org or on the school web site.
· All paperwork must be completed on final forms.
· Academic requirements will be set forth by East Hamilton School.
· We will begin practice on July 16th from 5:30 to 7:30. I will give out more information at a later date but just wanted to give you a start date. All paperwork must be completed before they can be on the field.
· All players must complete 5 days of acclamation before they can dress in full pads. The first two days are helmets only and the next three days are helmets and shoulder pads.
· Please join the Band app at https.//band.us/n/aaa1AeyeReeb7 because this is how I will send updates and share other information.
· Please understand that there may be changes so please continue to look at the web site and join the Band app for all updates.
· Tentative Practice Schedule
· July 16th and 17th 5:30-7:30 pm (shorts, t-shirts and helmets)
· July 21st_24th 5:30-7:30 pm (shorts, t-shirts, helmets, and shoulder pads)
· July 24th full pads if completed the 5 days of acclamation.
· July 28th – July 31st 5:30-7:30 pm
· August 4th – 7th 3-5 pm
· Players first day of school is August 6th
· Jamboree is on August 16th

Golf

Softball
East Hamilton Middle School Softball 2025
Preseason Conditioning, Tryouts, and Upcoming Season:
1. Preseason Conditioning will take place on the following dates: Tues 1/21, Wed 1/22, Thurs 1/23, Tues 1/28, Wed 1/29, Thurs 1/30 immediately after school and conclude at 4:00 pm daily (athletic apparel and gloves only).
2. Tryouts will take place February 3rd-4th immediately after school and conclude at 4:30 daily.
3. Team practice during the season will be immediately after school and conclude at 4:30 daily unless other notification is given.
4. Practice will take place at the softball field or in the gymnasium. ***Players must be academically eligible and have a physical, concussion, and cardiac forms uploaded to final forms before they try-out or participate in any way, including conditioning!*** ***Tryouts and practices are closed to parents, relatives, friends, etc.***
5. Softball participation fee will be $150 and will need to be paid by Monday, March 3rd.
6. An online store to order optional items for both players and parents/family will be made available.

EH Middle School team
Registration is now open until October 15th. The season starts October 1 – January 31st. Due to none of the schools having a pool all swimmers must join a year round club in the area for practice then we come together for our swim meets.
